Ilizarov soft-tissue distraction technique combined with debridement and dressing changes can effectively promote the healing of chronic limb wounds after popliteal artery injury, improve the limb salvage rate and limb function, representing a feasible treatment strategy.
Key Findings
Results
Of 29 chronic wounds in 24 patients with prior popliteal artery injury treated with Ilizarov soft-tissue distraction plus debridement and dressing changes, 28 healed.
Wound healing rate was 96.6% (28/29 wounds).
Retrospective analysis of 24 patients admitted between January 2023 and June 2025.
Wounds ranged from 3 cm×2 cm to 12 cm×9 cm.
Wound reconstruction was performed sequentially using debridement, continuous dressing changes, and Ilizarov soft-tissue distraction, among other methods.
Results
The limb salvage rate was 95.8%, with one amputation.
23 of 24 patients had successful limb salvage.
One patient (4.2%) underwent amputation due to severe ischemic necrosis of the limb caused by thrombosis.
Results
Patients achieved moderate-to-good functional scores at final follow-up.
Hospital for Special Surgery (HSS) score ranged 58-85 (mean, 72.4).
Causes: traffic accident in 16, mechanical injury in 6, fall from height in 2.
Time from injury to revascularization was 4-16 hours (mean, 9.6 hours).
18 wounds in 16 patients were persistent chronic wounds after injury; 11 wounds in 8 patients were recurrent muscle liquefaction wounds after soft-tissue healing.
Methods
The external fixation configuration for soft-tissue distraction was individually assembled according to wound location and morphology.
Distraction was combined with debridement and continuous dressing changes in a sequential protocol.
What This Means
This study looked at 24 patients who had long-lasting wounds on their limbs after a serious injury to the artery behind the knee (the popliteal artery), even after blood flow had been restored surgically. Some wounds never closed, and others closed but repeatedly broke down because of muscle liquefaction. Doctors treated them with cleaning of the wound, repeated dressing changes, and the Ilizarov soft-tissue distraction technique, in which an external frame gradually stretches the surrounding skin and soft tissue to close the wound.
Almost all wounds healed (28 of 29), and 23 of 24 limbs were saved. One patient required amputation because a blood clot caused severe loss of blood supply. After roughly two years of follow-up on average, patients had moderate-to-good limb function scores, and nearly all regained fine touch sensation, with the remaining two recovering protective sensation.
This research suggests the technique may be a feasible option for these difficult wounds. However, it was a small retrospective study without a comparison group, so it cannot show whether the technique works better than other treatments.
