Association between adherence to the 24-hour movement guidelines and the prevalence and mortality of osteoarthritis: A population-based study.

TL;DR

Adherence to 24-hour movement guidelines is associated with lower OA prevalence and markedly reduced mortality risk among affected individuals.

Key Findings

Meeting all 3 components of the 24-hour movement guidelines was associated with lower prevalence of osteoarthritis compared with meeting none.

  • Analysis based on 34,051 adults from the National Health and Nutrition Examination Surveys (1999-2018)
  • Adjusted odds ratio = 0.80 (95% CI: 0.64-1.00) for OA prevalence when meeting all 3 guidelines vs. meeting none
  • Multivariable logistic regression was used to evaluate OA prevalence
  • The 24-hour movement guidelines encompassed moderate-to-vigorous physical activity, sedentary behavior, and sleep time

Among individuals with osteoarthritis, meeting all 3 movement guidelines was associated with the lowest risk of all-cause mortality.

  • 3,594 participants with OA were linked to the National Death Index through 2019 for survival analyses
  • Adjusted hazard ratio for all-cause mortality = 0.34 (95% CI: 0.23-0.49) for those meeting all 3 guidelines compared with meeting none
  • Cox proportional hazards models were used to estimate adjusted hazard ratios
  • Greater adherence to movement guidelines was linked to a stepwise reduction in mortality risk

Among individuals with osteoarthritis, meeting all 3 movement guidelines was associated with the lowest risk of cardiovascular disease mortality.

  • Adjusted hazard ratio for cardiovascular disease mortality = 0.29 (95% CI: 0.13-0.61) for those meeting all 3 guidelines compared with meeting none
  • Cox proportional hazards models were used to estimate the adjusted hazard ratio
  • This represented the lowest cardiovascular disease mortality risk observed across adherence categories

Even partial adherence to the 24-hour movement guidelines was associated with significant survival benefits in individuals with osteoarthritis.

  • Meeting 1 or 2 of the 3 guidelines was associated with significant survival benefits relative to meeting none
  • The survival benefit followed a stepwise pattern with greater adherence associated with progressively lower mortality risk
  • This pattern was observed for both all-cause and cardiovascular disease mortality outcomes

The study used a population-based design linking NHANES survey data to national mortality records to assess both OA prevalence and survival outcomes.

  • Data were drawn from National Health and Nutrition Examination Surveys conducted between 1999 and 2018
  • Total analytic sample for prevalence analyses was 34,051 adults
  • A subset of 3,594 participants with OA were linked to the National Death Index through 2019 for survival analyses
  • The 24-hour movement guidelines included three components: moderate-to-vigorous physical activity, sedentary behavior, and sleep time
