Adding CRP and LAVI to a conventional clinical model provided substantial incremental prognostic information for 6-month heart-failure readmission in HFrEF, although external validation is required before clinical implementation.
Key Findings
Results
Thirty percent of the 150 clinically stable HFrEF patients were readmitted for heart-failure decompensation within 6 months.
Single-centre observational cohort of 150 patients: 75 ischaemic cardiomyopathy and 75 idiopathic dilated cardiomyopathy.
All participants completed 6-month follow-up and no deaths occurred.
45 patients (30.0%) were readmitted; the primary outcome was documented readmission for heart-failure decompensation within 6 months.
Results
Readmitted patients had significantly higher baseline NT-proBNP, fibrinogen, CRP and LAVI than non-readmitted patients.
NT-proBNP: 3058 (2019-4567) vs. 648 (229-1267) pg/mL.
Fibrinogen: 407.8 ± 59.0 vs. 331.3 ± 52.3 mg/dL.
CRP: 5.60 (3.86-7.82) vs. 2.12 (1.00-2.84) mg/L.
LAVI: 33.90 (32.70-34.50) vs. 30.90 (30.20-31.90) mL/m2; all q < 0.001.
Results
CRP and LAVI independently predicted readmission in the primary multivariable model.
Each CRP doubling: adjusted OR 16.16 (95% CI 4.62-56.51).
Each 1 mL/m2 increase in LAVI: OR 2.50 (95% CI 1.23-5.10).
Analyses used logistic regression alongside group comparisons, Spearman correlations, ROC analysis and bootstrap internal validation.
Results
Adding CRP and LAVI to a clinical model substantially improved discrimination for 6-month readmission.
Augmented model apparent AUC 0.962 and bootstrap-corrected AUC 0.944.
Clinical model apparent AUC 0.856 and bootstrap-corrected AUC 0.828.
Conclusions
The authors conclude that inflammatory and left-atrial remodelling measures should complement, not replace, established clinical risk factors, pending external validation.
The study frames inflammation and left atrial remodelling as complementary dimensions of residual risk in HFrEF.
The cohort was single-centre, and the authors state external validation is required before clinical implementation.
What This Means
This study followed 150 stable patients with heart failure with reduced pumping function (75 with ischaemic and 75 with idiopathic dilated cardiomyopathy) for six months. About 3 in 10 (45 patients) were readmitted to hospital for worsening heart failure. Those readmitted had higher blood levels of inflammation markers (CRP and fibrinogen), higher NT-proBNP (a standard heart-strain marker), and larger left atria relative to body size (measured as LAVI) at the start.
When the researchers combined these measures with usual clinical risk factors, CRP and LAVI each independently predicted readmission. A model including them distinguished patients who were and were not readmitted much better (bootstrap-corrected AUC 0.944) than a model with clinical factors alone (0.828).
This research suggests that routinely available inflammation tests and a heart ultrasound measure of left atrial size could add useful information for judging early readmission risk, alongside standard clinical assessment. However, the study was done at a single centre with a modest sample, and the authors say the results need to be confirmed in other populations before being used in practice.
