Prognostic Impact of EASIX According to the Timing of Assessment in Newly Diagnosed Multiple Myeloma Undergoing Autologous Stem Cell Transplantation.

TL;DR

EASIX measured at diagnosis was associated with both OS and PFS in transplant-eligible NDMM, whereas continuous pre-autoSCT EASIX showed no consistent association with either outcome.

Key Findings

Higher log2(EASIX) at diagnosis was associated with inferior overall survival after adjustment for ISS stage.

  • Adjusted HR 1.38 (95% CI 1.11-1.71; p = 0.0035).
  • EASIX at diagnosis was available for 92 of 99 patients.
  • Retrospective single-center cohort of transplant-eligible NDMM patients who underwent autoSCT in 2017-2023.
  • EASIX was log2-transformed and analyzed with Cox proportional hazards regression.

Higher log2(EASIX) at diagnosis was associated with shorter progression-free survival after adjustment for ISS stage, age, and sex.

  • Adjusted HR 1.41 (95% CI 1.14-1.74; p = 0.001).
  • The adjustment set for PFS included ISS stage, age, and sex.

Diagnosis EASIX showed good discrimination for overall survival.

  • Harrell's C-index was 0.78.
  • Time-dependent AUC values were sustained over follow-up.

Pre-autoSCT EASIX was not significantly associated with OS or PFS when analyzed continuously.

  • Cutoff-based analyses using maximally selected log-rank statistics did not give a consistent, robust prognostic signal.

A direct paired comparison showed a significantly stronger prognostic association of diagnosis EASIX with PFS than pre-autoSCT EASIX, but the between-timepoint difference for OS was not statistically significant.

  • The comparison was made between EASIX measured at diagnosis and at pre-autoSCT.
  • The authors conclude that EASIX timing merits evaluation in larger independent cohorts.

The study was a retrospective single-center analysis of 99 transplant-eligible NDMM patients, assessing EASIX at two timepoints.

  • EASIX was calculated at diagnosis and immediately before autoSCT.
  • Methods included Kaplan-Meier analysis, Cox regression, maximally selected log-rank thresholds, Harrell's C-index, and time-dependent AUC.

What This Means

This study looked at EASIX, a simple score calculated from routine blood tests (reflecting stress on the lining of blood vessels), in 99 people with newly diagnosed multiple myeloma who were eligible for and underwent autologous stem cell transplant. The researchers compared the score taken at diagnosis with the score taken just before transplant to see which better related to survival and to time without disease progression. The score at diagnosis was linked to outcomes: higher values went with shorter overall survival and shorter progression-free survival, even after accounting for disease stage and, for progression, age and sex. The diagnosis score also did a good job of separating patients with better and worse survival (C-index 0.78). In contrast, the score measured just before transplant did not show a consistent link with either outcome. A direct comparison found diagnosis EASIX was significantly stronger for progression-free survival, but the difference for overall survival was not statistically significant. This research suggests that when EASIX is measured may matter for its usefulness as a prognostic marker in myeloma. The study was retrospective, took place at a single center, and was small, so the findings need confirmation in larger independent groups before any practical use.
